FACILITY USE POLICY
FACILITY DESCRIPTION AND FEE SCHEDULE
A Facility Use Request Form is required for ALL facility use except worship services, sacraments and weddings. Please download, print and complete the form. The completed form is to be provided to the Church Secretary in order to secure the date for your event.
FACILITY USE REQUEST AND AUTHORIZATION FOR USE PROCESS
All non-church sponsored uses require the user to complete a Facility Use Agreement. Please download, print and complete the Facility Use Agreement linked below. Non-church sponsored events also require the user to provide a Certificate of Insurance evidencing liability coverage and naming St. Davids-Queenston United Church as an additional insured. Please follow the link to the MARSH CANADA EVENT LIABILITY INSURANCE PROGRAM/NON-PROFIT COMMUNITY GROUP INSURANCE PROGRAM to request a quote and submit your application. When prompted, please quote church insurance certificate CCX00103 to proceed and purchase third party liability insurance for your event. Additional documents required with the signed facility use agreement include the security deposit, a copy of the Certificate of Insurance; and, a copy of the liquor permit and Smart Serve certificate(s), if alcohol has been approved. The completed agreement, along with the other required documentation is to be provided to the Church Secretary prior to the event.
